About the Opportunity

The Department of Veterans Affairs is seeking a dedicated Care Coordinator to join our team at the Orlando VA Medical Center. This is a full-time, permanent position with a salary range of $70,361 - $144,580 per year.

Job Overview:

  • Deliver high-quality patient care and provide leadership in delivering and improving holistic care through collaborative strategies with others.
  • Collaborate effectively with other healthcare professionals to ensure comprehensive care for our patients.
  • Develop and implement plans to improve patient outcomes and experiences.

Responsibilities:

  1. Coordinate patient care and develop individualized care plans.
  2. Liaise with multidisciplinary teams to ensure comprehensive care for our patients.
  3. Maintain accurate records and reports.

Requirements:

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Bachelor's degree in nursing required.
  • Experience in providing care to patients with complex needs preferred.
