We are currently seeking an experienced Nurse Care Coordinator for our operations.

The Nurse Care Coordinator at CCG, under general supervision, is responsible for overseeing the implementation of our Population Health initiatives.

This role involves designing and maintaining systems that ensure continuity of care between our treatment programs and both somatic and behavioral healthcare providers, as well as developing processes that assist clients in cultivating healthy lifestyles and managing their health conditions.


Key Responsibilities:
1. Serve as a comprehensive care coordinator for assigned participants, assessing their needs and facilitating communication among care providers.
2. Lead practice transformation initiatives based on Population Health principles and regulatory requirements.
3. Collaborate with agency leadership to promote the Population Health philosophy and provide staff education.
4. Build and maintain relationships with primary and specialty care providers, including inpatient facilities and emergency departments.
5. Monitor Population Health performance and implement improvement strategies based on data analysis.
6. Design and develop wellness and prevention initiatives.
7. Utilize patient registries effectively to manage specific populations and enhance health outcomes.
8. Consult with specialty practices to identify data needs related to care coordination.
9. Oversee participant visits to ensure appropriate care delivery.
10. Conduct comprehensive assessments of participants' physical, emotional, psychosocial, and environmental needs, developing tailored care plans.
11. Facilitate health education sessions.
12. Evaluate and adjust care plans in collaboration with other disciplines.
13. Review laboratory and diagnostic results, reporting findings to the appropriate healthcare providers.
14. Identify significant clinical findings and intervene as necessary under supervision.
15. Initiate client education based on identified needs and document appropriately.
16. Support compliance monitoring for participants, including medication management.
17. Consult with behavioral health staff regarding participants' health conditions.
18. Attend treatment team meetings to coordinate care effectively.
19. Assist in liaising with medical providers for care coordination.
20. Provide training on medical conditions and treatments to participants and staff.
21. Report any risks to patient safety and adverse events to supervisors.
22. Implement policies and procedures consistently in all duties.
23. Foster effective communication and collaboration with team members.

Qualifications:
- Registered Nurse licensure in Maryland or eligibility through Compact state agreements.
- Proven teaching, coaching, and development skills.
- Experience in a consultative role and leading complex changes.
- Ability to foster a motivating environment for clinical staff.
- Strong interpersonal, verbal, and written communication skills.
- Ability to collaborate with multidisciplinary teams.
- Knowledge of the Maryland healthcare landscape preferred.
- Proficiency in Microsoft Office and electronic medical records.
