Role Overview:


This position operates as a provider within the interdisciplinary team of Home-Based Medical Care (HBMC), directly delivering medical services to a designated patient population. Committed to enhancing patient health and well-being, this role collaborates with primary care physicians, HBMC team members, and other healthcare professionals to develop and implement individualized care plans throughout the patient’s journey.


Key Responsibilities:

  • Practices in compliance with state regulations governing advanced practice nursing.
  • Establishes and nurtures effective relationships with patients, caregivers, interdisciplinary team members, and collaborating healthcare providers.
  • Conducts comprehensive assessments for newly enrolled patients and provides continuous care thereafter. Develops and updates patient management care plans as necessary.
  • Completes follow-up assessments according to established protocols.
  • Prescribes necessary diagnostics and interventions to prevent unnecessary hospital admissions.
  • Demonstrates proficiency in fundamental nursing procedures, including IV placements, blood draws, and wound care, with training provided.
  • Consults with clinical teams following patient transfers to ensure continuity of care.
  • Educates patients and caregivers on disease management and wellness promotion.
  • Provides guidance and support to team members as appropriate.
  • Actively participates in meetings focused on patient care and clinical excellence.
  • Implements quality initiatives to uphold high standards of care and enhance care management.
  • Available for on-call duties during business hours and participates in after-hours on-call rotations.
  • Stays informed on relevant medical and nursing advancements through ongoing education and professional development.
  • Performs additional duties as assigned.
