Role Overview:


This position functions as a provider within the Home-Based Medical Care (HBMC) interdisciplinary team, focusing on the direct provision of home-based medical services to a designated patient population. You will work collaboratively with primary care providers, interdisciplinary team members, and health professionals to develop and implement individualized care plans throughout the patient’s healthcare journey.


Key Responsibilities:

  • Adhere to state regulations governing advanced practice nursing.
  • Build and maintain effective relationships with patients, caregivers, interdisciplinary team members, and other healthcare providers.
  • Conduct comprehensive assessments for newly enrolled patients and provide ongoing care, updating care plans as necessary.
  • Perform follow-up assessments in accordance with established protocols.
  • Prescribe necessary diagnostics and interventions to prevent unnecessary hospital admissions.
  • Demonstrate proficiency in basic nursing procedures, including IV placement, blood draws, and wound care.
  • Coordinate with clinical teams during patient transfers.
  • Educate patients and caregivers on disease management and wellness promotion.
  • Support and educate staff as needed.
  • Participate in meetings focused on patient care and clinical excellence.
  • Implement quality initiatives to enhance care management and delivery.
  • Be available for on-call duties during business hours and participate in after-hours on-call programs as required.
  • Stay updated on relevant medical and nursing research through continuing education and professional development.
  • Perform additional duties as assigned.

Required Qualifications:

  • Completion of an accredited Nurse Practitioner program.
  • Current Nurse Practitioner license in the State of New Jersey (ANP, FNP, or GNP credentials).
  • A minimum of 2 years of clinical experience in primary care, home-based medical care, geriatrics, or emergency care.
  • Knowledge of long-term care and community resources.
  • Reliable transportation access.


Preferred Qualifications:

  • Familiarity with CPT, ICD-10, and HCPC coding.
  • Understanding of clinical standards of care.
  • Awareness of NCQA requirements and CMS guidelines.
