Role Overview:


This position functions as a provider within the Home-Based Medical Care (HBMC) interdisciplinary team, directly delivering home-based medical services to a designated patient population. Committed to enhancing patient health and well-being, this role collaborates with primary care physicians, HBMC team members, and, when necessary, health plan Medical Directors and other healthcare professionals to establish and implement patients' care objectives throughout their healthcare journey.


Key Responsibilities:

  • Practices in compliance with state laws and regulations governing advanced practice nursing.
  • Establishes and nurtures effective relationships with patients, caregivers, interdisciplinary team members, primary care providers, and facility staff as applicable.
  • Conducts comprehensive assessments for all newly enrolled patients and provides ongoing care. Develops and updates patient management care plans as necessary based on changes in condition or post-hospitalization.
  • Completes follow-up and post-discharge assessments according to established protocols.
  • Prescribes necessary diagnostics and interventions to prevent unnecessary acute admissions.
  • Demonstrates proficiency in basic nursing procedures, including IV placement, blood draws, injections, foley catheter insertion, nasal packing, and wound care. Training and skills assessments are provided for new hires and annually thereafter.
  • Consults with hospital and emergency clinical teams following patient transfers.
  • Educates patients and their caregivers about disease processes and effective management strategies, promoting overall wellness.
  • Provides guidance, support, and education to staff as appropriate.
  • Actively participates in meetings focused on patient care and clinical excellence.
  • Implements quality initiatives and HEDIS measure campaigns to uphold the highest standards of care and enhance care management.
  • Available for on-call responsibilities during business hours and participates in the clinical after-hours on-call program on a rotational basis.
  • Stays informed on relevant medical and nursing research, technology, and related issues through continuing education, professional meetings, and journal reviews.
  • Performs related duties as necessary and other assigned tasks.

Required Qualifications:

  • Completion of an accredited Nurse Practitioner program.
  • Current Nurse Practitioner license in the State of New Jersey (ANP, FNP, or GNP credentials).
  • Minimum of 2 years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, or emergency/urgent care.
  • Knowledge of long-term care, community resources, and cost-effective patient care delivery systems.
  • Access to reliable transportation.


Preferred Qualifications:

  • Familiarity with CPT, ICD-10, and HCPC coding.
  • Understanding of clinical standards of care.
  • Awareness of NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts.
