Client Services Coordinator

Role Responsibilities:
This position plays a crucial role in supporting corporate goals by leveraging expertise in Long-Term Services and Supports (LTSS) to deliver effective Case Management.
- Engage in case discussions with supervisory personnel.
- Allocate approximately 75% of your time visiting members in their residences or long-term care settings.
- Exhibit flexibility, adaptability, and patience in diverse situations.
Key Duties Include:
- Conducting intake, screening, referral, assessment, and reassessment processes.
- Developing and managing support plans, monitoring member health and welfare, and documenting all interactions and case management activities in the designated system.
- Maintaining confidentiality of patient information in accordance with company policies.
- Performing regular monitoring contacts as mandated, ensuring face-to-face interactions with members at least biannually.
- Updating assessments and care plans to reflect any changes in member conditions or services.
- Overseeing the delivery of services outlined in the Support Plan and Prior Authorization Requests.
- Participating in training sessions provided by the department and agency.
- Ensuring compliance with CMA Contract Agreement requirements, delivering accurate and timely work.
- Reporting any suspected cases of mistreatment or abuse to the appropriate authorities.
- Taking on special projects and assisting with system assessments as necessary.
- Traveling as required, with reliable transportation at your own expense.
Qualifications:
- A Bachelor's degree in a relevant human behavioral science field (e.g., human services, nursing, social work, psychology, public health) or equivalent experience.
- Minimum of 5 years of relevant experience in LTSS.
- Proficiency in MS Office applications, including Word, Excel, and Outlook.
- Access to reliable transportation and willingness to travel up to 75% within the assigned area.
- Residence in designated regions.
Preferred Experience:
- Previous experience as a caseworker or case manager within the LTSS population may substitute for educational requirements on a year-for-year basis.
Compensation:
The hourly wage for Colorado residents ranges from $23.22 to $45.43, based on various factors including local labor markets and individual qualifications. UnitedHealth Group adheres to all applicable minimum wage laws.
