Care Coordinator

Job Summary:
The University of Vermont Health Network seeks a skilled Care Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for coordinating activities related to a specific patient population and providing direct patient care for a specific unit and shift.
Key Responsibilities:
- Function as a leader of multidisciplinary teams, providing and coordinating care to patients and families.
- Concurrently utilize and review services, consult with patients and families, and promote continuity of care.
- Provide preceptorship and committee work as required at this level of practice.
Requirements:
- Current RN licensure or compact licensure recognized by the State of Vermont is required.
- The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter.
- Three years of relevant RN experience.
