Position Overview
As a vital member of Avantara Elgin, the Social Services Coordinator plays a crucial role in enhancing the quality of life for our Guests. This position is classified as exempt and reports directly to the Director of Social Services.
Key Responsibilities
The Social Services Coordinator is responsible for ensuring that the emotional and social needs of our Guests are addressed in accordance with applicable regulations. Key functions include:
- Maintaining accurate and up-to-date documentation of each Guest's activities in compliance with federal, state, and local standards.
- Creating detailed social histories and psychosocial assessments that reflect the Guests' strengths, preferences, and cultural backgrounds.
- Providing orientation to Guests and their families regarding facility services, limitations, and rights.
- Participating in care conferences to identify and address the psychosocial needs of Guests.
- Assisting with the coordination of admissions, transfers, and discharges within the facility.
- Communicating facility rules and regulations to families and addressing any inquiries.
- Overseeing the inventory and labeling of Guests' personal belongings.
- Engaging in interdisciplinary team meetings to assess and supervise psychosocial group activities.
- Completing mandatory documentation within required timelines.
- Offering regular consultations to Guests to discuss and resolve any challenges they may face.
- Mediating conflicts among Guests, families, and staff.
- Connecting Guests with community resources to meet their needs.
- Collaborating on comprehensive discharge plans with Guests, families, and multidisciplinary teams.
- Providing informal counseling to help Guests engage in social activities.
- Maintaining an organized filing system for the Social Services Department.
- Reporting any hazardous conditions or equipment issues promptly.
- Adhering to safety protocols while performing tasks.
- Ensuring the comfort, privacy, and dignity of Guests through respectful interactions.
- Responding promptly to calls for assistance in Guest care areas.
- Participating in departmental meetings and training sessions as required.
- Contributing to the organization's Quality Assurance and Performance Improvement (QAPI) initiatives.
- Engaging in compliance training and adhering to established policies.
Work Environment
This role is situated within a healthcare setting, requiring regular movement throughout the facility. Standard office equipment will be utilized, and there may be occasional exposure to Guests with contagious illnesses.
Physical Requirements
The position primarily involves sedentary work, with some filing duties that may require lifting and bending.
Qualifications
Applicants should possess a minimum of two years of experience in a social work capacity, preferably within a healthcare environment. A bachelor's degree in social work or a related field is preferred, along with five years of relevant experience.
