Position Overview

OBJECTIVE OF THE ROLE

Under the supervision of the Director of Nursing, the Nursing Supervisor is responsible for assigning, planning, implementing, and evaluating care across multiple units. This role serves as a mentor and directs nursing personnel in all clinical areas, ensuring effective nursing practices. Responsibilities include scheduling, guiding, and instructing both professional and support nursing staff in their duties.

Key Responsibilities:

Nursing Process

• Oversee and assess nursing care through comprehensive evaluation methods, including documentation and adherence to established standards.

• Analyze trends in nursing care delivery and propose necessary innovations or modifications.

• Monitor patient conditions and initiate appropriate nursing interventions, including direct care.

• Collaborate with the Director of Nursing to coordinate and evaluate new initiatives aimed at enhancing care delivery.

• Support nursing staff in organizing patient care and ensuring continuity based on clinical needs.

• Participate in pre-visit planning and daily clinical discussions to uphold Patient Centered Medical Home Standards.

• Identify care gaps and collaborate with the team to address them through outreach and education.

• Engage in care coordination activities through Accountable Care Organizations (ACO) and contribute to healthcare delivery initiatives.

• Maintain clinical expertise and serve as a resource for nursing staff across all sites.

• Facilitate quality assurance standards and participate in chart reviews to enhance nursing practices.

• Organize and evaluate orientation programs for new nursing staff, ensuring effective clinical training.

• Identify needs within the service area through observation and evaluation.

• Address staff needs and resolve issues as they arise.

Leadership and Management

• Provide supervision across designated units.

• Act as a resource for unit staff and manage multiple units as assigned.

• Offer leadership and direction aligned with departmental objectives.

• Foster open communication within the unit and across departments.

• Utilize sound judgment and problem-solving skills in decision-making.

• Ensure appropriate staffing patterns to meet patient needs and manage scheduling effectively.

• Consult with superiors regarding significant situations requiring attention.

• Conduct performance evaluations and provide constructive feedback to staff.

• Promote educational development for unit personnel and direct patient/family education efforts.

• Assist in the recruitment and orientation of new staff members.

Safety and Compliance

• Ensure the safety and comfort of patients and staff.

• Participate in daily safety huddles and collaborate with support services to maintain a secure environment.

• Uphold policies regarding infection control and emergency procedures.

Professional Development

• Commit to personal continuing education and professional growth.

• Attend relevant meetings and workshops to enhance knowledge and skills.

• Serve on professional committees to contribute to nursing and facility-wide initiatives.

Personnel Responsibilities

• Adhere to facility policies and maintain professional conduct.

• Provide direct patient care as needed across clinical areas.

• Complete annual training and comply with policies governing patient rights and confidentiality.

• Fulfill additional duties as assigned by Nursing Administration.

Minimum Qualifications
1. Valid New York State RN license and current registration.
2. Certification in Basic Life Support (BLS) through the American Heart Association (AHA).
3. Minimum of three years of experience as a registered nurse.

Preferred Qualifications

CERTIFICATIONS/LICENSES:

• NYS RN License

• Ambulatory Nursing certification preferred

EDUCATION:

• Master’s degree in nursing or related field required

• Commitment to ongoing professional development.

EXPERIENCE:

• Two years or more in primary care

• One year of ambulatory nursing preferred

• Two years of supervisory experience
