We're seeking a skilled Field Case Manager RN to join our Optum At Home product team. As a key member of our interdisciplinary care team, you will help patients navigate the healthcare system and connect them to essential support services. By providing preventive care, we can empower patients to stay well at home.

The ideal candidate will have 3+ years of experience in a hospital or acute care setting, as well as a current, unrestricted RN license in Washington D.C. A valid driver's license and reliable transportation are also requirements. In this field-based role, you will be working closely with members, caregivers, and healthcare professionals to develop personalized care plans.

Salary Information:

The salary range for this role is $58,300 to $114,300 annually, based on factors including local labor markets, education, work experience, certifications, and more. In addition to your salary, you'll enjoy a comprehensive benefits package, incentive programs, equity stock purchase, and 401(k) contribution.

Job Responsibilities:

  • Assess, plan, and implement individualized care strategies for patients
  • Manage members through acute medical situations and chronic conditions
  • Identify, initiate, and follow up on referrals for community-based programs and providers
  • Communicate with stakeholders to ensure quality coordinated care and services
  • Advocate for patients and families as needed

Required Qualifications:

  • Current, unrestricted RN license in Washington D.C.
  • 3+ years of experience in a hospital or acute care setting
  • Demonstrated ability to type and navigate a Windows-based environment
  • Valid driver's license and reliable transportation
  • Ability to work in a field-based capacity in Washington, D.C.
  • Must live within 50 miles of Washington D.C.
