We are seeking a skilled LPN to join our team at Watseka Rehabilitation and Health Care Center for a 13-week contract.
Job SummaryThis LPN position involves providing high-quality patient care in a skilled nursing facility setting. The ideal candidate will have excellent communication skills, a strong work ethic, and the ability to work effectively in a team environment.
Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes.
To be successful in this role, you will need:
- A current LPN license in the state of Illinois.
- At least 1 year of experience in a skilled nursing facility setting.
- Excellent communication and interpersonal skills.
- A strong commitment to providing high-quality patient care.
