We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Chas CO LTD. As a Patient Care Coordinator, you will play a critical role in improving the overall health of our communities by supporting patients with complex chronic conditions.
Key Responsibilities- Analyze and Identify Patients in Need of Support: Utilize available registries and reports to identify patients with complex chronic conditions, and implement appropriate care plans to ensure optimal health outcomes.
- Conduct Patient Chart Reviews and Interviews: Perform thorough patient chart reviews and conduct patient interviews to identify clinical and social determinants of health barriers, and develop personalized care plans to address these challenges.
- Coordinate Care and Support Services: Collaborate with patients, healthcare teams, and outside entities to coordinate care and support services, ensuring seamless transitions and optimal health outcomes.
- Support Patient Care Teams: Work closely with patient care teams to create and implement care plans, and provide education and support to patients and their families.
- Track and Present Outcomes: Monitor and track patient outcomes, and present data to leadership to inform quality improvement initiatives and optimize care delivery.
- Education: Bachelor's degree in nursing from an accredited school of nursing or equivalent experience required.
- License and Certification: Registered Nurse (RN) license in the State of Washington required upon hire, and eligible to obtain RN license in Idaho.
- Experience: Community health setting experience preferred, with experience in chronic disease management and electronic health records.
- Skills: Excellent customer service, oral and written communication skills, compassion for patients, and ability to work well in a team environment.
The Patient Care Coordinator will be required to stand, sit, and be mobile for a significant portion of the workday, with frequent use of hands to finger, handle, or feel. The position will also require occasional climbing, balancing, stooping, kneeling, or crouching, as well as communicating by talking and hearing.
