About the Role

We are seeking a highly skilled Pharmacist to join our team at OnePoint Patient Care. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring the safe and effective use of medications.

Responsibilities
  • Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
  • Maintain accurate patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
  • Ensure timely ordering, evaluation, and discussion of laboratory tests for high-risk conditions with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
  • Inform the Pharmacy Manager of medication-related errors and assist with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
Qualifications
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
