Overview
As a key member of our healthcare team, the Practice Nurse is responsible for delivering comprehensive nursing services to a diverse patient population. This role encompasses care for patients across various age groups, including pediatric, adolescent, adult, and geriatric.

Position Details

WORK SCHEDULE: Full-Time (40 hours/week); primarily Monday to Friday, day shift from 7:30 AM to 5:00 PM, with one evening shift weekly until 8:30 PM and one Saturday per month from 7:30 AM to 12 PM.

Sign-on Bonus: $1,500 for eligible external candidates.

Tuition Support: 100% Tuition Assistance available at PA College of Health Sciences.

KEY RESPONSIBILITIES:
Qualified candidates will be expected to perform the following essential functions:
  • Deliver clinical guidance via phone or electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards.
  • Provide tailored patient education on preventive care, chronic disease management, acute conditions, medications, and other relevant topics.
  • Prepare patients for appointments by utilizing appropriate rooming standards.
  • Administer prescribed injections and medications in compliance with departmental protocols and regulatory guidelines.
  • Assist healthcare providers during various procedures, minor surgeries, and examinations as directed.
  • Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
  • Collect and prepare laboratory specimens for further analysis.
  • Authorize prescription refills as per provider instructions.
  • Support tracking of tests and monitor overdue results.
  • Perform suture removal and dressing changes as required.
  • Ensure exam rooms are properly stocked and sanitized after each patient visit, maintaining cleanliness of medical instruments and equipment.
  • Collaborate with providers and patients to review and adjust care plans, assessing patient progress and following up as necessary.
  • Engage in proactive outreach for patients needing preventive and chronic disease services.
  • Facilitate care coordination among specialists and manage transitions of care to gather all necessary patient information.
  • Assist in coordinating referrals, educational resources, and community services.
  • Actively participate in initiatives aimed at enhancing patient experience and quality of care.
  • Join daily huddles with the healthcare provider.
  • Serve as an advocate for patient experience, maintaining professionalism in all interactions with patients, visitors, and colleagues.
  • Handle patient information confidentially in accordance with privacy policies.
ADDITIONAL RESPONSIBILITIES:
The following tasks are considered secondary to the primary responsibilities:
  • Perform various administrative duties as needed.
  • May involve some regional travel; a valid driver's license is required.
  • Participate in on-call duties as necessary.
  • Other responsibilities as assigned.
MINIMUM QUALIFICATIONS:
  • Completion of an LPN or RN educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) from the PA State Board of Nursing.
  • AHA CPR/BLS certification required within 30 days of hire.
PREFERRED QUALIFICATIONS:
  • 1-2 years of nursing experience in a primary or specialty care setting.
  • Previous experience in triage, emergency care, cardiac care, intensive care, or related nursing specialties.
