Job Summary:
The Hospice RN utilizes a comprehensive understanding of nursing to deliver care to clients in their homes or appropriate community settings. This role is inclusive of all age groups and diagnoses, emphasizing a holistic approach that integrates family, environmental, and community resources to enhance the well-being of both clients and their families. The focus is on care management, particularly in end-of-life situations. The Hospice RN is well-versed in hospice care, particularly in pain and symptom management, and employs the nursing process to assess, diagnose, plan, and evaluate care.
Essential Duties:
Conduct assessments and develop care plans through discussions with patients and their primary caregivers, performing thorough physical evaluations and integrating this with health and psychosocial histories. Manage cases and document skilled care, treatments, and diagnostic procedures in accordance with physician directives and nursing standards. Take responsibility for primary nursing management of assigned patients, ensuring continuity of care through effective communication with relevant personnel. Complete initial evaluations for assigned patients, assessing their status and nursing needs upon admission and during subsequent visits. Prepare recertification and discharge summaries, along with all federally mandated data entry and forms. Regularly assess patient needs, informing physicians and supervisors of any changes in condition, and adjust treatment plans as necessary. Make referrals as appropriate. Identify and document the need for supplies and teaching materials for upcoming visits. Maintain a daily time log with supporting documentation in electronic records, ensuring synchronization with the home system as required. Utilize knowledge of regulations pertaining to Home Health/Hospice. Participate in on-call duties during evenings and weekends as scheduled. Demonstrate a thorough understanding of hospice programs, philosophies, and standards of care. Organize weekly patient schedules and enter them into the electronic system by the designated time. Manage patient assignments with a productivity target of five visits per workday, considering geographic and patient acuity factors. Exhibit proficiency in the use and maintenance of laptop computers. Engage in at least one Home Health/Hospice committee. Attend a majority of staff meetings and all mandatory training sessions. Complete annual education requirements through online or in-service training, earning the necessary continuing education units. QualificationsLicense/Certification:
Current RN license issued by the State of Oregon. Must obtain American Heart Association BLS for Health Care Providers within the first month of employment. Possession of a valid Oregon Driver's License and automobile insurance is required.Experience:
Minimum of two years of acute care nursing experience is required. Preferred experience includes one year in hospice nursing or two years in home health nursing. Flexibility in scheduling and the ability to meet on-call requirements are essential. Strong assessment, communication, and documentation skills are necessary.Education:
An Associate's Degree in Nursing is required.
