Position Summary: As a Community Health Worker (CHW), you will serve as a vital link between Medicaid members and the healthcare system, ensuring that they receive the appropriate care and support. Your role will involve conducting home and social assessments, providing education, and assisting members in navigating their healthcare journey. This position requires a genuine interest in working with individuals facing complex medical, addiction, and behavioral health challenges.

Work Schedule: This is a full-time position (40 hours/week) with a standard schedule from Monday to Friday, during normal business hours. The role combines telephonic communication with fieldwork, allowing for flexibility in a home-based office setting.

Key Responsibilities:
  • Foster positive relationships with members to enhance their healthcare experience.
  • Assist members in setting health goals and developing personalized care plans.
  • Proactively engage with members to encourage self-management of their health.
  • Support members in maintaining their well-being and reducing hospital visits.
  • Facilitate access to mental health and substance use treatment as needed.
  • Ensure members receive their prescribed medications.
  • Educate members about community resources and available benefits.
  • Conduct follow-up activities post-hospital discharge and connect members to social services.
  • Encourage member participation in work or volunteer opportunities to strengthen social connections.
  • Collaborate with the care team, including community providers and internal staff.
  • Stay informed about community cultures and values to better serve members.

Qualifications:
  • High School Diploma / GED or equivalent community outreach experience.
  • Minimum of 1 year of field-based experience.
  • Knowledge of community resources and cultural values.
  • Must reside within a reasonable commuting distance.
  • Valid Driver's license and reliable transportation.
  • Ability to travel locally as required.
  • Proficient in computer applications, including Microsoft Outlook and Teams.
  • Designated quiet workspace at home to secure Protected Health Information (PHI).
Preferred Qualifications:
  • Associate degree in a health-related field.
  • LPN Licensure or CNA/HHA certification.
  • Community Health Worker Accreditation.
  • Experience in Managed Care settings.
  • Familiarity with Medicaid/Medicare populations.
