Job Title: Children's Care Manager

About the Role:

BHSN is seeking a passionate and dedicated Children's Care Manager to join our team. As a key member of our organization, you will play a vital role in providing outreach and enrollment services to children presumed to meet eligibility requirements of New York State's Children's Health Home program.

Key Responsibilities:

  • Provide family-driven, youth-guided services to children and families
  • Document all case activity, including outreach, consent development and assessment, plan development, client progress and transition arrangements
  • Administer CANS-NY assessment and update periodically
  • Develop comprehensive plans of care and inventory and coordinate existing services relevant to the plan
  • Convene and conduct quarterly meetings to review progress and update plans
  • Provide education to families and children concerning services and conditions being treated
  • Assist families and children in acquiring and maintaining public benefits
  • Provide monthly Health Home services for children based on acuity level as per DOH requirements

Requirements:

  • Bachelor's degree in human services field with 2 years' relevant experience or Master's degree in human services field with 1 year of relevant experience
  • Relevant experience is defined as serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services
  • Excellent engagement and organizational skills
  • Ability to work independently and collaboratively
  • Valid NYS driver's license with driving record acceptable to BHSN insurance carrier required
