Job SummaryThe Foster Care Case Manager plays a vital role in ensuring the safety and well-being of children and families in foster care. This position requires a strong understanding of child welfare principles, excellent communication skills, and the ability to work effectively with diverse stakeholders.
Key Responsibilities- Facilitate Family Support Team meetings to ensure seamless delivery of services to children and their families.
- Maintain a caseload of 12-15 children, providing full case management services from assignment to case closure or transfer.
- Document all activities and maintain data entry in FACES and CSWIS online database systems in a timely and accurate manner.
- Engage in face-to-face contact with assigned children and families at least monthly, as well as regular telephone contact with support providers to review case progress.
- Evaluate and report on the impact of services to team members and judiciary, attend court hearings, and schedule and facilitate FST meetings within prescribed timeframes.
- Bachelor's degree in social work, psychology, counseling, education, or a related human services field.
- Minimum of two years of relevant case management experience.
- Valid driver's license and risk-free driving record.
- Ability to undergo initial and periodic screenings for child abuse/neglect, criminal history, sexual offender registry, and child care/elder care disqualification list.
