Key Responsibilities:

  • Deliver in-home direct patient care in accordance with the prescribed care plan while closely monitoring patient progress and recovery.
  • Evaluate and document patient conditions, reporting any significant changes as necessary.
  • Administer medications, including insulin and IV fluids, ensuring thorough documentation.
  • Manage wound care, including dressing changes and assistance with personal hygiene.
  • Assess patients for potential injuries, infections, or bedsores, providing appropriate treatments.
  • Collaborate with healthcare providers, aides, and allied health professionals to achieve optimal patient outcomes.

Qualifications:

  • Completion of a practical nursing program and possession of a current LPN/LVN license.
  • Current CPR/AED/BLS/First Aid certification.
  • Reliable transportation to various care sites.
  • Experience with trach and/or ventilator care is preferred but not mandatory.
