Job Summary

Brockton Area Multi-Servi is seeking a skilled Licensed Practical Nurse to join our team in a community care setting. The ideal candidate will possess a strong background in nursing care and physical rehabilitation, with a focus on providing high-quality care to individuals with complex medical needs.

Key Responsibilities:

  1. Administer medications and treatments in accordance with Department of Public Health's Medication Administration Program (MAP) policies and other funder regulations.
  2. Complete daily written notes, data tracking, and skilled assessments to ensure accurate documentation of care provided.
  3. Develop and implement individualized care plans that promote growth, dignity, and independence for individuals served.
  4. Collaborate with direct care and supervisory teams to ensure the health status of individuals is met.
  5. Provide oversight of meals, ensuring food and meals are prepared properly, and train staff on meal preparations and specialized feeding protocols as needed.
  6. Assist with the maintenance of medication books, house log books, etc. to ensure program compliance with all regulations.
  7. Ensure medications, medical and safety supplies are ordered and available as needed.
  8. Follow admission and discharge requirements for hospitalized individuals.
  9. Provide training to both staff and individuals served in relevant areas (e.g., health, hygiene, safety, diet, etc.).
  10. Attend staff meetings and provide coverage at day habilitation programs or other residential programs as needed.
  11. Ensure safety standards and infection control protocols are followed.

Requirements:

  • Possession of Massachusetts license as a nurse.
  • Valid driver's license in state of residence and 3 years of driving experience.
  • Basic computer literacy, including the ability to record data, use electronic time and attendance software, and receive and send email.

Preferred Qualifications:

  • 2 years experience working with individuals with acquired brain injury or similar disabilities.

Knowledge/Skills/Abilities:

  • Knowledge of the theories, methods, procedures, and practices of nursing care and physical rehabilitation as applicable to community residential services.
  • Ability to lift, pull/push, transfer, maneuver, and reposition 25 pounds.
  • Ability to reach, bend, stand, sit, and walk; perform fine motor activities.
