About the Role

We are seeking a skilled Patient Care Assistant - Home Health to provide high-quality care and support to patients in their homes. As a Patient Care Assistant, you will work closely with our clinical team to develop personalized care plans and deliver exceptional patient outcomes.

Your Responsibilities

  • Provide basic care and personal assistance to patients, such as bathing, dressing, and grooming.
  • Assist patients with daily activities, such as meal preparation and light housekeeping.
  • Monitor patient vital signs and report any changes to the clinical team.
