Responsibilities:

* Work with patients, families, medical staff, and community agencies to determine the best course of care
* Assess post-hospital care needs and provide necessary interventions for effective clinical resource management
* Provide crisis intervention and psychosocial assessment
* Collaborate with healthcare teams to ensure seamless care transitions

Requirements:

* Graduate of an accredited Registered Nursing School (RN) with a current GARN license
* One (1) year previous experience in case management, utilization review, or social services
* Excellent oral and written communication skills
* Ability to type accurately at an approximate rate of 40-45 wpm

Preferred Qualifications:

* BSN
* Three (3) years' experience in case management, social services, or related field
* Certification as Certified Case Manager
