Job Summary

The Case Manager is a critical role at Woods Services, responsible for developing, implementing, and monitoring program planning processes for assigned caseloads of individuals. This position requires a strong understanding of health insurance and benefits eligibility, as well as the ability to communicate effectively with individuals, families, and staff.

Key Responsibilities
  • Own the case and all paperwork related to the individuals on their caseloads from pre-admission to discharge.
  • Ensure that paperwork is individualized, submitted according to deadlines, and meets all regulatory requirements.
  • Collaborate with day, clinical, and health services departments to ensure seamless care coordination.
  • Provide regular presence in programming areas and serve as a support to individuals and staff.
  • Confer with families and guardians to ensure informed decision-making and effective communication.
Requirements
  • Master's degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities.
  • OR Bachelor's degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities.
  • Valid driver's license and reliable transportation.
  • Ability to lift a minimum of 50 pounds.
