Job Title: RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Trinity Health. As a RN Case Manager, you will be responsible for assessing, planning, evaluating, and coordinating patient care, including discharge planning and utilization needs.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Coordinate with the interprofessional team to meet patient care goals
  • Establish partnerships with site Patient Experience champions to align efforts with patient satisfaction outcomes
  • Identify and eliminate barriers to effective patient stay management and promote improvements in work processes within the department, hospital, and community
  • Provide consultation to professional colleagues and employees regarding patient stay options, financial considerations, psychosocial concerns, risk management, and ethical issues
Requirements:
  • Valid license as an RN
  • BSN required, Master's preferred
  • 3-5 years Case Management experience, hospital or community based
  • BLS certification required
