Job Description
This is an exciting opportunity for a Senior Care Coordinator to join our team at the Harbor Community Health Center-Plymouth.
Key Responsibilities:
- Assist in implementing programs to ensure the proper management of High-Risk Patient populations (Substance Use Disorder, Infectious Disease, Complex Care, Geriatric, and Chronic Conditions)
- Develop and implement comprehensive plans of care in conjunction with the team and/or providers
- Provide health education and disease management to patients and families
- Participate in clinical tracking programs to ensure compliance with clinical outcome measures, prevention, and follow-up care
- Triage patients as needed and support transitions of care
- Document all patient interactions and care plans in the Electronic Medical Record (EMR)
Requirements:
- Degree in Nursing (RN) required; Bachelor's Degree in Nursing (BSN) preferred
- At least 2 years of nursing experience in a community health setting
- 1 year of experience in medical case management or managing complex patients
- Excellent customer service skills and ability to work effectively with diverse customers
- Ability to work within a team setting and independently

The estimated salary range for this position is $65,000 - $85,000 per year, depending on qualifications and experience.
