POSITION SUMMARY

The Community Reintegration Specialist is responsible for facilitating access to community resources, ensuring continuity of care post-release, advocating for client needs, and serving as a liaison among various systems that affect individuals with mental health challenges, co-occurring substance use disorders, and chronic health conditions.

MINIMUM QUALIFICATIONS

  • A bachelor's degree in Mental Health or a related discipline.
  • At least one year of experience in the social services sector.
  • Experience in Case Management.
  • Preference will be given to candidates with a background in correctional health care.
