Job Summary:

The Registered Nurse Case Manager plays a vital role in developing and implementing care plans for patients. This position requires strong communication and organizational skills to ensure seamless patient care.

Key Responsibilities:

  • Collaborate with physicians and healthcare professionals to create comprehensive care plans.
  • Conduct regular assessments to monitor patient progress and identify potential barriers to care.
  • Develop and implement strategies to address patient needs and goals.
  • Maintain accurate and up-to-date patient records.

Requirements:

  • Montana Licensure (Registered Nurse)
  • American Heart Association BLS
  • Minimum of 2 years of nursing experience

Preferred Qualifications:

  • Bachelor's Degree in Nursing
  • 2 years of acute care hospital experience as a case manager

Essential Job Functions:

As a Registered Nurse Case Manager, you will be responsible for:

  • Completing comprehensive assessments of patients per regulated timelines.
  • Developing and implementing case management plans in collaboration with patients, caregivers, and healthcare professionals.
  • Performing ongoing monitoring of care plans to evaluate effectiveness and suggest changes as needed.
  • Maintaining ongoing patient case loads for regular outreach and management.
