Case Manager, Individuals Position

The Case Manager: Individuals is an integral part of the Customer Support team at Human Resource Development Council District IX, Inc. This role is responsible for delivering direct outreach and case management services to individuals experiencing homelessness, severe mental illness, or co-occurring disorders. The Case Manager collaborates with internal teams, external partners, and community agencies to ensure customers receive comprehensive, trauma-informed support while strongly focusing on equity, confidentiality, and client autonomy.
Key Responsibilities
• Deliver street outreach and case management services to assist clients in developing housing goals and accessing needed services.
• Collaborate with Compliance Specialists to determine program eligibility and benefit levels by identifying and requesting necessary documentation.
• Work closely with all agency programs, including the compliance department, to uphold best practice standards in case management.
• Engage in ongoing training opportunities on best practices, regulatory requirements, and organizational policies.
Requirements
• Bachelor's degree in Psychology/Social Services/Education or related field, from a four-year college or university preferred or comparable life/work experience.
• Valid State Driver License required.
• Reliable vehicle and appropriate liability insurance are required.
Working Conditions
The incumbent works in a temperature-controlled office environment and operates a motor vehicle under all weather conditions. The position requires exposure to customers with unfavorable body odor and those in crisis, which can manifest in loud outbursts, anger, and threats of harm to self or others.
