About the Role:

The Rehabilitation Unit Care Manager is a critical position at MaineGeneral Health, responsible for optimizing the quality and continuity of care for patients. This role focuses on promoting positive health outcomes and patient satisfaction.

Key Responsibilities:

  • Investigate, assess, and plan interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Interview and assess patients and/or their family, caregivers, and/or legal representatives to determine, prioritize, provide, and/or arrange for needed internal and external services/interventions.
  • Consult with multidisciplinary teams regularly to evaluate patient status and appropriateness of medical care, including coordination of care in all care centers.
  • Participate in discharge planning, including identification and coordination of appropriate resources, and provide discharge education and resource referrals to patients when necessary.
  • Perform chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

Requirements:

  • Maine state licensure as a Licensed Social Worker (LSW), Occupational Therapist (OT), or Registered Nurse (RN) is required.
  • Excellent teamwork, communication, and collaboration skills.
  • Proven and demonstrated critical thinking skills.
