We are seeking a highly skilled and compassionate Nurse Navigator to join our team at Confluence Health. As a key member of our cardiology department, you will play a vital role in coordinating care for patients with cardiac conditions, ensuring seamless transitions throughout their treatment journey.
Key Responsibilities- Develop and implement individualized care plans for patients with cardiac conditions, in collaboration with physicians and the healthcare team.
- Provide comprehensive nursing care, including symptom management, education, and support to patients and their families.
- Facilitate communication and coordination among healthcare providers, patients, and families to ensure timely and effective care.
- Assess patients' social needs and connect them with community resources and social services as needed.
- Collaborate with the healthcare team to determine the need for further diagnostic work-ups and coordinate necessary tests or procedures.
- Ensure effective utilization of resources and efficient patient throughput along the continuum of care.
- Current licensure in the state of Washington (RCW or licensure through Multistate Nurse Licensure Compact).
- Current BLS Health Provider and ACLS certification.
- Competent in basic computer skills.
- 3 years of clinical experience.
- Ability to work independently and perform role with minimal guidance.
- Previous department experience.
This role requires a combination of working independently and close communication with providers. Hours may be irregular at times, and overtime may be required.
