Key Responsibilities- Provide case management services to 15-20 youth and families, ensuring their needs are met and goals are achieved.
- Develop and implement individualized plans to address the unique needs of each youth and family.
- Collaborate with community resources and agencies to provide comprehensive support services.
- Conduct regular home visits and assessments to monitor progress and identify areas for improvement.
- Maintain accurate and detailed records of case management activities and outcomes.
- Bachelor's degree in a social service discipline, such as social work, psychology, or counseling.
- Experience in case management or a related field, preferably with youth and families.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with youth, families, and community partners.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
- Valid driver's license and reliable vehicle, with liability insurance.
