Job Summary

We are seeking a skilled and empathetic Mental Health Specialist to join our team. As a key member of our Community Support Team, you will play a pivotal role in delivering mental health and substance use services to help clients achieve their recovery goals and improve overall well-being.

Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough assessments to identify client strengths, needs, and goals, ensuring a personalized approach to care.
  • Develop and Implement Treatment Plans: Collaborate with clients, their families, and our team to develop and implement tailored treatment plans, monitoring progress and making adjustments as needed.
  • Provide Direct Therapeutic Interventions: Offer individual, family, and group therapy, as well as crisis intervention, to support clients in achieving their recovery goals.
  • Coordinate Community Resources: Link clients to necessary community resources, ensuring they receive comprehensive support.
  • Advocate for Clients: Advocate for clients within various systems to ensure they receive the support they need.
  • Monitor Progress and Adjust Treatment Plans: Regularly monitor client progress and adjust treatment plans to ensure effectiveness.
  • Maintain Accurate Documentation: Maintain detailed documentation in accordance with agency and regulatory standards.
  • Collaborate with Team Members: Work closely with our Community Support Team to ensure a holistic approach to client care.
Additional Responsibilities
  • Provide Psychoeducation: Offer psychoeducation as indicated, supporting clients in their recovery journey.
  • Assist with Crisis Interventions: Assist with crisis interventions and develop relapse prevention and disease management strategies.
  • Coordinate Assessment Activities: Coordinate assessment activities and ensure the implementation of the person-centered plan (PCP).
  • Consult with Medical and Non-Medical Providers: Consult with medical and non-medical providers, engaging community supports and incorporating their input into the planning process.
  • Link Clients to Clinically Appropriate Services: Link clients to clinically appropriate services and arrange for necessary evaluations.
  • Complete Functional Needs Assessments: Complete functional needs assessments to determine service scope and anticipated outcomes.
Qualifications
  • A Bachelor's degree in a related field.
  • At least 2 years of experience working with individuals with mental health or substance use challenges, preferably in a community-based setting.
  • Strong knowledge of evidence-based practices in mental health and substance use treatment.
  • Excellent communication, organizational, and problem-solving skills.
  • Commitment to person-centered care and cultural competence.
  • Valid driver's license and reliable transportation.
