As a vital member of the cardiac care team, the Cardiac Care Nurse Specialist oversees the management of outpatient advanced heart failure patients, collaborating closely with heart failure cardiologists. This role involves participating in outpatient consultations and employing critical analysis to interpret clinical indicators, abnormal lab results, and pulmonary artery waveforms, particularly in relation to CardioMems devices. The specialist ensures seamless transitions for patients throughout the healthcare continuum while maintaining effective communication with patients, families, and the entire care team.

KEY RESPONSIBILITIES:

  • Work in conjunction with physicians to coordinate and implement cardiac clinical strategies.
  • Assess and prioritize patient inquiries requiring advanced clinical judgment and timely intervention.
  • Manage patients reliant on inotropic medications, serving as a liaison with home infusion nursing staff and pharmacists to adjust medications based on laboratory findings.
  • Conduct assessments and evaluations in the outpatient clinic alongside physicians, ensuring accurate and updated documentation in the electronic health record system.
  • Oversee anticoagulation management for heart failure patients, including bridging protocols for surgical procedures.
  • Educate patients and their families on heart failure management, medication regimens, and dietary considerations.
  • Participate in inpatient rounds with the Advanced Heart Failure physician to enhance patient education, care transitions, and discharge planning.
ADDITIONAL DUTIES AND COMPETENCIES:
  • Review and interpret incoming laboratory results, addressing any abnormalities.
  • Ensure completion of necessary documentation for disability, home care, hospice, and medical clearances.
  • Facilitate direct admissions for patients as needed, both planned and urgent.
  • Engage in weekly meetings to discuss candidacy for heart transplant and VAD patients.
  • Manage Cardiomems patients regarding diuretic adjustments and follow-up lab work, requiring proficiency in interpreting pulmonary artery pressure waveforms.
  • Coordinate genetic testing for patients with specific cardiomyopathies, ensuring timely specimen processing.
  • Screen daily MCOT reports for arrhythmias, promptly reporting urgent findings to physicians.
  • Enroll patients in cardiac rehabilitation programs and assist in scheduling outpatient procedures, including pre-procedure laboratory tests and instructions.
  • Participate in the ongoing evaluation of potential research candidates, managing data and communication with healthcare providers and clinical subjects.
  • Arrange for intravenous iron and diuretic infusions through the designated infusion center.
  • Complete medication prior authorizations for heart failure and amyloidosis treatments.
  • Travel to satellite clinics to support healthcare providers as needed.
  • Oversee care transitions from inpatient to outpatient settings, collaborating with a multidisciplinary team.
  • Interact with colleagues, patients, and visitors in alignment with the values of Jefferson Health.
QUALIFICATIONS:

EDUCATION AND TRAINING:
Graduation from an accredited nursing program is required; a Bachelor of Science in Nursing (BSN) or commitment to complete a BSN within three years of hire is strongly preferred.

CERTIFICATIONS AND LICENSES:
Active Registered Nurse license in the Commonwealth of Pennsylvania is required; licensure in New Jersey is preferred. Current ACLS certification is mandatory. CITI Certification must be obtained within six months of hire. CHFN or CHF-N certification through AAHFN is preferred.

EXPERIENCE:
A minimum of three years of experience with heart failure patients is strongly preferred. Experience in a cardiac physician practice, cardiovascular lab, or at least two years in a hospital nursing environment is highly desirable. Background in critical care services is also preferred.

ADDITIONAL INFORMATION:
Familiarity with basic cardiac rhythms and knowledge of Swan-Ganz waveform interpretation is beneficial.
