Position Title: Care Coordinator Assistant at United Homecare Services

Company Overview: United Homecare Services is a community-focused non-profit organization committed to delivering non-medical, licensed in-home care solutions for the elderly.

We are currently in search of a Care Coordinator Assistant for our operations. This role is ideal for individuals with at least one year of supervisory experience in healthcare or social services, particularly with senior citizens. The successful candidate will exhibit excellent communication and caregiving abilities, possess strong organizational skills, and demonstrate empathy towards the elderly and their family caregivers. A state-level background check is a prerequisite for all applicants.

Key Responsibilities:

  • Work autonomously as well as collaboratively within a team environment
  • Maintain professional communication through various channels including email, text, and in-person interactions
  • Manage incoming calls, provide timely information, and conduct consultations in clients' homes
  • Utilize the ClearCare database to manage schedules for clients and caregivers
  • Assign caregivers to clients based on their specific skills and requirements
  • Facilitate introductions between caregivers and clients, ensuring proper training is provided
  • Regularly assess and update client service plans as necessary
  • Be available for on-call duties 24/7, including one weekend each month
  • Assist with caregiver responsibilities during emergencies on short notice
  • Travel within a designated service area, with additional mileage compensation
  • Maintain accurate agency records, including electronic data and files for clients and employees
  • Ensure that all necessary forms and care documentation are readily accessible
  • Carry out additional office tasks as assigned

This role offers the opportunity to work part-time, with a commitment of 20 hours per week, Monday to Friday during daytime hours.
