Role of the Case Manager:

The Case Manager is a clinical expert responsible for overseeing patient cases, utilizing a strong foundation of clinical knowledge and skills. This role may also involve delivering certain therapeutic services to patients. The Case Manager ensures quality care by developing, implementing, managing, and assessing patient care plans.

Key Responsibilities:
  • Develop, implement, manage, and evaluate individualized patient care plans;
  • Gather clinical data, analyze this information, and provide insights to the treatment team;
  • Effectively communicate medical criteria and clinical details between insurance providers and the treatment team as necessary;
  • Compile clinical data for administrative purposes as required.
Qualifications:

Education, Skills, and Experience Required:
  • Bachelor's degree in Social Work, Counseling, or a related Human Services field.
  • 1-2 years of experience in a behavioral health environment, with a preference for experience with the specific population served by the clinic.
  • Prior experience in case management and/or utilization review is preferred.
