Job Summary:

The Laurels Recovery Center is seeking a skilled Discharge Planner to join our team. As a Discharge Planner, you will be responsible for responding to the overall mental health, substance abuse, or co-occurring needs of individuals in our program. This includes facilitating educational and treatment groups, reviewing case management and referral needs, and developing comprehensive discharge plans.

Key Responsibilities:
  • Facilitate educational and treatment groups to individuals in services
  • Review case management and referral needs during the course of treatment
  • Identify discharge planning needs and develop comprehensive discharge plans
  • Meet with program participants to identify discharge planning needs
  • Assist with case management and referral needs
  • Provide transportation to needed resources and service coordination with other providers

Requirements:
  • Bachelor's Degree in Human Services field and QMHP preferred with CSAC or CSAC-A preferred
  • Master's Degree in Human Services field and/or licensure preferred
  • Extensive knowledge of community and regional resources
  • Experience providing group treatment services
  • Ability to effectively intervene in a crisis situation
  • Good written and oral communication skills
  • Knowledge of local human service resources
  • Ability to use independent judgement and discretion in the assignment of duties

About Us:
The Cumberland Mountain Community Services Board (CMCSB) is a Chapter 10 Community Services Board under Virginia law. We provide mental health, intellectual disabilities, and substance abuse treatment services to the residents of Buchanan, Russell, and Tazewell Counties. We operate over twenty locations, including outpatient offices, mental health recovery programs, group homes, and day support programs for intellectually disabled individuals.
