**Job Summary**

We are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in evaluating the needs of our clients and developing personalized care plans to ensure their safety and well-being.

**Key Responsibilities**

  • Conduct thorough assessments of client needs and develop comprehensive care plans in collaboration with clients, caregivers, and families.
  • Coordinate and initiate services provided by our network of healthcare professionals and community resources.
  • Advocate for clients to access benefits and services for which they are eligible.
  • Maintain accurate and up-to-date case records in compliance with regulatory requirements and OCES standards.
  • Perform initial home visits to determine program eligibility and develop service plans.
  • Participate in interdisciplinary activities, including collaboration with our Transition Support Team.
  • Provide backup support for our Information & Referral Specialist, handling general intake and providing information and referral services to clients and applicants.

**Requirements**

  • Bachelor's degree in social work or a related field.
  • Current Social Work License preferred.
  • One year of experience in the social services field, preferably in home care or a related field.
  • Considerable knowledge of social work principles and the social, emotional, and physical processes of aging.
  • Ability to obtain sensitive information through interviews, draw appropriate conclusions, and make suitable recommendations.
  • Ability to respond effectively to emergency situations involving elder abuse and/or neglect.
  • Excellent communication and writing skills, with proficiency in computer use.
