Renal Care Specialist

A leading dialysis management company, SBKC Management, seeks an experienced Renal Care Specialist to join its team.
Estimated Salary: $70,000 - $90,000 per annumKey Responsibilities:- Evaluate and develop comprehensive nutrition assessments and plans in accordance with CMS regulations.
- Collaborate with patients and interdisciplinary teams to establish and update individualized care plans.
- Monitor nutritional status, lab results, and dialysis adequacy to inform appropriate interventions and recommendations.
- Design and deliver personalized nutrition education and counseling based on patient needs and goals.
- Document progress notes regularly and conduct thorough assessments upon admission, 90 days, and annually.
- Participate in quality improvement initiatives and departmental meetings as needed.
- Provide coverage for Nutrition Services at assigned facilities during leaves of absence, vacations, or census increases.
- Degree in renal dietetics or related field.
- Current certification in renal nutrition or equivalent experience.
- Excellent communication and interpersonal skills.
- Able to travel to assigned facilities.
