Job SummaryWe are seeking a skilled Care Coordinator RN to join our interdisciplinary team. The successful candidate will work closely with physicians, family members, nurses, and other hospital team members to provide compassionate quality care and concurrent assessment. The Care Coordinator RN will assist patients and families from pre-admission through discharge planning to ensure appropriate utilization of services in accordance with the Utilization Management Plan.
Key Responsibilities- Formulate a plan of care to meet identified needs in the most appropriate setting at the time of initial assessment.
- Perform utilization review function within 24-48 hours of patient admission to the hospital and document same on internal worksheet.
- Interview patient and/or family with risk criteria for education needs and any special discharge concerns and continue to follow through discharge.
- Review plan of care with healthcare team and patient/family.
- Attend interdisciplinary rounds and provide information to staff regarding Millman Care Guidelines, payor guidelines, and identified variances in the care plan.
- Provide information regarding denials and LOS to medical staff.
- Coordinate the plan of service that is identified for effective hospital stay and discharge planning.
- Work closely with Social Workers and other members of the Interdisciplinary Care Team to effect a timely discharge.
- Diverse clinical experience in both Med/Surg and Critical Care a plus.
- ~3-5 years clinical hospital experience required.
- Must be able to make decisions, work independently, and communicate effectively with Interdisciplinary teams, patient, families, and physician staff to formulate a safe discharge plan.
