We are seeking a skilled and compassionate RN Homecare Coordinator to join our team at Marie Lawrence Associates. As a key member of our home healthcare team, you will provide comprehensive nursing care to our participants in their homes and centers.

Job Summary:

The RN Homecare Coordinator will work under the direction of the Center Director to provide individualized care plans, educate participants on disease management and medication, and coordinate services with contracted resources. This is a full-time position, Monday through Friday, 8:30am to 5:00pm, with 90% field-based travel throughout Hudson County.

Responsibilities:
  1. Perform initial assessments, annual reassessments, and change of condition assessments of participants in their home environment.
  2. Develop and coordinate individualized plans of care with appropriate resources.
  3. Provide education and instruction to participants on care and management of their disease and medications.
  4. Direct and oversee the coordination of care and services for participants under your care.
  5. Act as a liaison between contracted services and our client, in conjunction with the Director of Nursing.
Requirements:
  1. LSW or RN License - New Jersey.
  2. Current driver's license and proof of auto insurance.
Skills:
  1. Broad knowledge base of health care problems of the frail elderly.
  2. Knowledge of common safety hazards in the home and improvements to establish a safer environment.
  3. Skilled in identifying problems and recommending solutions.
  4. Knowledge of home care services, nursing principles, and practices with particular reference to the elderly.
  5. Ability to perform physical assessments and develop plans of care.
  6. Knowledge of health instruction techniques and practices.
  7. Skilled in preparing and maintaining records and written reports and responding to correspondence.
  8. Excellent organization and documentation skills.
  9. Familiarity with community health resources available to participants and families.
  10. Skilled in developing and maintaining department quality assurance.
  11. Knowledge of current NJ Home Health policies.
  12. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members, and family caregivers.
  13. Ability to react calmly and effectively in emergency situations.
  14. Work well with all members of the care team.
  15. Knowledge of elder abuse and reporting.
  16. Familiar with Dementia, Wandering Behavior, and Intervention.
  17. Knowledge of Standard Precautions & Fire and Emergency Procedures in healthcare settings.
  18. Knowledge of Participant Rights and Workplace Safety.
Experience:
  1. Minimum of 1 year of nursing experience in an acute care setting or 1 year experience in home care or long-term care setting.
  2. Minimum of 1 year working with the frail or elderly.
