Key Responsibilities:
  • Conduct thorough assessments of members' needs, identifying areas for improvement and developing personalized care plans.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
  • Monitor members' progress, making adjustments to care plans as needed to optimize health outcomes.
  • Provide education and support to members and their families, empowering them to take an active role in their healthcare.
  • Develop and maintain relationships with healthcare providers, ensuring timely and effective communication.

Requirements:
  • Current, unrestricted RN license in applicable state(s) required.
  • 3-4 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.

  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
