Job Title: LPN-Mobile Crisis Services

Job Summary:

Elwyn is seeking a skilled Licensed Practical Nurse (LPN) to join our Mobile Crisis Services team. As a member of our interdisciplinary team, you will provide critical care and support to individuals in crisis, working closely with other healthcare professionals to ensure seamless care coordination.

Key Responsibilities:

  • Conduct thorough nursing evaluations at the site of the encounter, gathering essential medical history, current concerns, medications, and recent hospitalizations.
  • Collaborate with clients' current mental health resources, therapists, and other members of the Interdisciplinary Team (IDT) to plan and implement therapeutic programs of care.
  • Present pertinent data obtained through direct observation, including client interviews, physical evaluations, and reports from external resources, to the IDT to communicate client progress and ensure continuity of care.
  • Administer and document medications, ensuring necessary consents have been completed and documented prior to administration.
  • Document all behavioral and physical changes in the client's status and other pertinent information in nursing progress notes and electronic health records.
  • Order medical and nursing supplies, restock as needed, and maintain medical equipment in proper working order.
  • Demonstrate flexibility, adaptability, and teamwork when nursing assignments must be modified to ensure program compliance and quality of care to clients.

Requirements:

  • Licensed as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
  • One (1) year of psychiatric, medical surgical, or chemical dependency nursing experience.
  • Knowledge and understanding of program regulating bodies (e.g., Office of Mental Health & Substance Abuse Services OMHSAS, Department of Behavioral Health) preferred.
  • Ability to work in a fast-paced environment, manage and prioritize multiple priorities, and demonstrate strong attention to detail.
  • Excellent customer interaction, collaboration, presentation, and written and verbal communication skills.
  • Current CPR certification and ability to perform CPR, first aid, crisis intervention, medication administration, and crisis intervention techniques using agency-trained protocols.
