Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Mobile Crisis Team. As an LPN on our team, you will play a critical role in providing crisis intervention and stabilization services to individuals in need.

Key Responsibilities
  • Conduct focused nursing evaluations at the site of the encounter, including assessing medical history, current medical concerns, medications, and recent hospitalization.
  • Collaborate with clients' current mental health resources and/or primary care resources to plan and implement therapeutic programs of care.
  • Document all details of the encounter, including client progress and continuity of care, in the electronic health record.
  • Administer and document medications as indicated, ensuring necessary consents have been completed and documented prior to administration.
  • Report any observed medication and treatment side effects or abnormal laboratory values to the Registered Nurse (RN) immediately for further observation and follow-up care, if needed.
Requirements
  • Licensed as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
  • One (1) year of psychiatric, medical surgical, or chemical dependency nursing experience.
  • Knowledge and understanding of program regulating bodies, such as OMHSAS and the Department of Behavioral Health, preferred.
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities.
  • Demonstrated strong attention to detail, time management, and organizational skills.
  • Excellent customer interaction, collaboration, presentation, and written and verbal communication skills.
  • Current CPR certification and ability to perform CPR, first aid, crisis intervention, medication administration, and crisis intervention techniques using agency-trained protocols.
Working Conditions

The LPN will work in a fast-paced environment, managing and prioritizing multiple, often competing, priorities. The ability to work effectively as part of a team and maintain confidentiality of client records is essential.
