Job Overview

The Lead Physical Therapy Specialist is responsible for delivering exceptional physical therapy services in alignment with the physician's care plan.

Key Responsibilities:

1. Deliver physical therapy interventions based on the physician's established care plan.

2. Collaborate with the physician to assess the patient's functional capabilities through appropriate testing and provide insights for care plan adjustments.

3. Conduct thorough evaluations of patients to create or modify treatment plans utilizing diagnostic tests for muscle, nerve, joint, and functional abilities.

4. Implement therapeutic activities aimed at alleviating pain, restoring function, and maximizing patient performance while accommodating their limitations.

5. Monitor, document, and communicate patient responses to treatments and any changes in their condition to the physician or care coordinator.

6. Assess the patient's home environment and provide safety recommendations as necessary.

7. Procure and/or create adaptive devices and instruct patients on their safe and effective use.

8. Educate patients, family members, and home health staff on physical therapy processes and the safe use of assistive devices.

9. Demonstrate and teach alternative methods for daily living activities, mobility, and positioning.

10. Regularly evaluate rehabilitation needs with patients and coordinate with care teams to ensure comprehensive care plans.

11. Refer patients to other specialists as needed and document the rationale for any decisions made.

12. Facilitate interdisciplinary coordination by participating in patient care meetings.

13. Provide training and guidance to patients and caregivers on home care practices relevant to the patient's condition.

14. Maintain accurate and timely patient care documentation in accordance with organizational policies.

15. Conduct field evaluations for therapists as required by regulatory standards.

16. Achieve productivity standards based on operational needs.

17. Support the Director in quality improvement initiatives and educational programs.

18. Perform initial assessments for specific cases as required.

19. Undertake additional responsibilities as needed.

Minimum Qualifications:

1. A Bachelor's Degree from an accredited institution and a valid New York State license to practice Physical Therapy.

2. A minimum of four years of professional experience in physical therapy under medical supervision, including at least two years in a supervisory role.
