Job Summary

We are seeking a skilled Licensed Vocational Nurse to join our team at Evergreen Life Services Texas. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our clients, ensuring their health and well-being in a safe and therapeutic environment.

Key Responsibilities
  • Contribute to the planning and implementation of daily projects to promote the health and well-being of our clients.
  • Update the Medical Program Coordinator and other staff on health-related issues and provide input on individual support plans.
  • Assist in educating staff on the conditions of our clients and participate in their education and training.
  • Assess client health needs and situations that may pose health risks, documenting findings in nurse notes.
  • Participate in medical quality assurance checks and contribute to the compilation of monthly summaries on client health.
  • Monitor and report seizures to the RN Healthcare Coordinator and assist in seizure data gathering.
  • Serve as the liaison nurse for clients, assisting the Medical Program Coordinator in making nursing decisions and judgments.
  • Demonstrate knowledge of infection control procedures and communicate with physicians as needed.
  • Accompany clients to physician visits, obtain necessary information, and route orders back to the Medical Program Coordinator.
  • Administer medications according to best practice guidelines and provide skilled nursing treatments such as tube feeding and wound care.
  • Review and maintain incoming physician orders and communicate new orders to the oncoming shift nurse.
Requirements
  • Graduate of an approved Licensed Practical Nursing Program.
  • Minimum of one year of clinically relevant experience, preferably working with elderly or intellectually disabled individuals.
  • Knowledge of medical nursing procedures, first aid, and basic principles of nursing.
  • Ability to work in a variety of settings and environments, both indoors and outdoors.
  • Good driving record and reliable transportation.
