Role: RN/LMSW/LPN - Case Manager in Emergency Services

The RN/LMSW/LPN Case Manager is tasked with delivering comprehensive nursing care to a designated group of patients during their stay. This role encompasses assessing patient and care team requirements, formulating, executing, and reviewing an effective Plan of Care, and adjusting it as necessary based on evolving patient needs.

The Case Manager also oversees the utilization review process, ensuring adherence to established standards of care to confirm medical necessity, appropriate care levels, and necessary notifications.

Additionally, the Case Manager facilitates patient transitions across the care continuum, ensuring safe and timely movement to the appropriate care level.


Key Responsibilities:

  • Assess hospital admissions using evidence-based criteria to confirm that care is medically justified and provided at the most suitable level.
  • Ensure that patient information, including demographics, insurance details, primary care provider, and advanced directives, is accurately maintained in the electronic medical record (EMR).
  • Evaluate the needs of the emergency department patient population and arrange necessary outpatient follow-ups.
  • Stay informed about organizational and departmental goals and take proactive steps to achieve desired outcomes.
  • Maintain up-to-date knowledge of federal, state, and accreditation standards related to utilization review and discharge planning.
  • Be visible and approachable to patients and staff.
  • Adapt to additional responsibilities as required.

Qualifications:

Education: Completion of an accredited Nursing program or a Master's in Social Work.

Must obtain Case Management Certification or an equivalent within 18 months of employment.

Experience: A minimum of 2 to 3 years in an acute care hospital environment or relevant healthcare case management experience.

Proficiency in understanding community resources, alongside strong communication, organizational, critical thinking, and problem-solving skills.

Familiarity with EMR systems and computer literacy is essential.

Demonstrates resilience and emotional stability in high-pressure situations, with the organizational skills necessary to thrive in a dynamic healthcare setting.

Licensure: Current, unrestricted NY State Licensure as RN/LPN/LMSW.


Preferred Qualifications:

  • BSN in nursing is preferred.
  • Prior experience in Utilization Review is advantageous.
  • Case Management Certification is preferred.

Physical Requirements:

  • Ability to stand, sit, or move for extended periods. Capability to navigate various units, transport oneself to different locations, and perform data entry into EMR and other data management systems.

Work Environment:

  • This position requires onsite presence.

Compensation:

  • Annual salary range: $77,230.46 to $95,450.40.
