Job Summary

We are looking for a compassionate and detail-oriented Registered Nurse to work as a Case Manager in our Home Health Care setting. As a Home Health RN, you will be responsible for managing patient cases, conducting skilled nursing visits, and providing education to patients and their families.

Responsibilities

  • Conduct skilled nursing visits to adult and geriatric medical/surgical patients in their homes or senior living facilities
  • Manage patient cases, including assessing patient needs, developing care plans, and implementing interventions
  • Provide education to patients and their families on disease management, medication management, and other health-related topics
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Document patient information and progress in a timely and accurate manner

Requirements

  • Current RN license in the state
  • At least 2 years of licensed RN experience in any setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to travel to patient homes and senior living facilities
