Responsibilities- Conduct thorough health assessments, including medical history, physical examination, and diagnostic testing, to diagnose and manage common acute and chronic conditions.
- Develop and implement individualized care plans, providing education and guidance to patients and their families on health maintenance, disease prevention, and management of chronic conditions.
- Collaborate with our healthcare team to provide comprehensive care, including referrals to specialists and coordination of care with other healthcare providers.
- Provide clinical oversight and guidance to NP/PA trainees, NP residents, and new providers, ensuring they receive the support and resources needed to succeed in their roles.
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that promote high-quality patient care and safety.
- Stay up-to-date with the latest medical research, best practices, and technological advancements, applying this knowledge to improve patient outcomes and enhance the overall quality of care.
- Board certification as a Nurse Practitioner from a recognized certifying organization.
- Current medical license to practice in the State of California, including DEA license and CPR certification.
- Minimum of three years of experience as a Nurse Practitioner, with a strong background in primary care and community health.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
