Responsibilities:
Conduct adult/geriatric assessments, medical history, diagnosis, and treatment
Provide health education and collaborate with physicians, case managers, and other healthcare professionals
Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements:
Masters degree in nursing or equivalent experience
Current RN and NP licensure, with prescription authority or ability to obtain
Board certification in a Matrix-approved specialty
1 year of experience as a Nurse Practitioner
Strong computer skills and familiarity with electronic health records
