Job Summary

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at Tiburcio Vasquez Health Center. As a key member of our multidisciplinary team, you will provide high-quality, patient-centered care to our diverse patient population.

Responsibilities
  • Conduct thorough health assessments, including medical history and physical examinations, to diagnose and manage common acute and chronic conditions.
  • Develop and implement individualized care plans, incorporating preventive screening and health education to promote healthy behaviors and disease prevention.
  • Collaborate with other healthcare providers to ensure comprehensive care and coordinate services as needed.
  • Provide clinical oversight and guidance to NP/PA trainees, NP residents, and new providers.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure high-quality patient care.
  • Stay current with evidence-based practices and maintain certification in areas relevant to the role.
Requirements
  • Minimum of three years of experience as a Nurse Practitioner required.
  • Board certification from a certified Nurse Practitioner or related program.
  • Current medical license to practice in the State of California, including DEA license, and CPR certification.
  • Previous experience providing medical care within a community health setting preferred.

  • Strong knowledge of medical principles and practices.
  • Excellent communication and interpersonal skills to build trust and respect with patients, staff, and colleagues.
  • Ability to work effectively in a fast-paced, dynamic environment and prioritize tasks to meet deadlines.
  • Commitment to maintaining confidentiality and adhering to organizational policies and procedures.
  • Willingness to adapt to changing circumstances and priorities.
